I Mrs. Ruth Jensen have been employed at the Schultz Rest Home for over 9 years.

I wish to be discharged from the State Hospital at Lincoln, Nebraska.

I wish to leave this place of employment. I also wish for my social security check to be mailed to me in my own name - Ruth Edith Jensen.

If any problems come up I would like to talk to my brother at Blair, Nebraska. His name is Mr. Garfield Norby. And no out-siders allowed to send me to any institution.

I will support my-self and pay my own bills.
