Arrived in Phoenix by bus Oct. 21st, 1968 and went to see Dentist the next day. Had ex-ray pictures taken of my teeth.

Thursday Oct 24, 1968.

Left about 10 minutes to 12 - Bus didn't come then called from service station for taxi cab. Returned 10 minutes past 2 P.M. Pulled 3 - .

About 5 minutes to 1 Dr. Tate's office lady called and asked how I was feeling. I said all right. She reminded me about my appointment Monday at 2 o'clock.

Oct 28th had 1 tooth - back one and dentist pulled it in 3 parts.

Nov. 4, 1968

I had 2 front teeth pulled.

Nov. 6, 1968.

I went to see dentist and impresstions made for dentures.

Nov 15th went to dentist and had impress ions made for dentures.

Nov. 20th 1968 - went to the dentist and had dentures fited.

Dec. 6th

Left Phoenix at 11:45 Thursday. Had breakfast at Neetle, California at 10 minutes to 4. In Las Vegas at about 10 minutes to 7 in the morning. Stoped at Baily, Nevada for 10 minutes and had refreshments.

Stoped at town in Nevada. Small town and ate 3:30.

Stoped at another little town and had coffee (Hawthron)

Arrive Reno 5 minutes after 5 P.M.
